Request permanent deletion of your MyPath.Care account — or, if you have a legal basis to have particular information removed, request that instead.
How a deletion request works
You submit this form
We only need the email address on the account. Nothing is deleted at this step.
We confirm it is you
Someone from our team emails the account address to verify the request. A request nobody confirms is never acted on.
We delete, and tell you
Deletion completes within 30 days of your confirmation, and we email you when it is done.
How a removal request works
You tell us exactly what and why
Name the information precisely and give the legal basis. A request we cannot locate, we cannot act on.
We confirm it is you
We email the account address before acting, the same as for a deletion request.
We review, route, and reply
Where an organization controls the record, the request goes to them. We reply with the outcome either way.
Why some information stays
MyPath.Care is a shared record. A care case is worked by an adjuster, a case manager, a provider and others at once, and the organizations coordinating it are required to keep the record of what happened. Deleting your account removes you — it does not erase the case history other parties are relying on, or that the law requires them to retain.
